Nancy Hickman, BSW, MA, CMC
Director of Client Services

With 25 years in human services, Nancy has experience in many
aspects of geriatric care management. Her skills range from advocacy
and counseling to management of client services. Nancy holds a
master’s degree in public administration and is a Certified Care Manager.

Eileen Nevitt, LCSW, ACSW, CMC
Geriatric Care Manager Supervisor

Combining psychosocial aspects of chronic illness with geriatric care since 1976, Eileen has worked in community-based as well as hospital settings, e.g., Northwestern University Medical Center. She facilitates local support groups (ALS, stroke) and serves on the stroke support group advisory board. Eileen holds a master’s degree in social service administration.

Nancy Chaknova, MSW, LCSW, CMC
Geriatric Care Manager Supervisor

With more than 25 years of experience, Nancy has directed geriatric services for Jewish & Family Services, Satellite Senior Housing, and Masonic Home for Adults. She serves on the Alameda County Multi-Disciplinary Team for Elder Abuse. Nancy holds a master’s degree in social work.

Eileen Zagelow, BA, CMC
Geriatric Care Manager Supervisor

Eileen has been in the healthcare field all of her professional life. She has been with Eldercare Services since 1993 and specializes in Alzheimer’s and dementia care issues. Eileen provides extensive community education in dementia.

Christa Petermann, CMC

Christa has worked in social services for the past 16 years. After working in skilled nursing, she became a Care Manager and has been with Eldercare Services since 1991.

Mary Tierney, BA, CMC

Mary started in care management as a social worker in New York City, and brings more than 10 years of experience to clients and their families. She has been with Eldercare Services since 1997 and specializes in dementia.

Julianna Dickey, LCSW, MSW, CMC

Juli has worked in various aspects of geriatric care and psychotherapy for more than 25 years. She has experience in nursing homes and day treatment programs, both in direct patient care and as a program supervisor.
