Geriatric Care Management: What is a Geriatric Care Manager?

“You can't imagine the peace of mind you have given me during this time. Thanks for weathering the ups and downs, for your enduring patience, and for helping me navigate through all the challenges I've faced. You provide a wonderful service not only to the people you care for directly, but also to the families.”

Guiding Families

Professional Care Managers are the foundation of our work in geriatric care and are unmatched in the industry. Geriatric Care Managers are professionals educated in nursing, gerontology, social work, psychology, or related fields. A Care Manager is an objective party that helps families make difficult decisions. Most have advanced degrees and are licensed in their field. Our Care Managers’ experience includes an average of 18 years serving older adults and their families, with some exceeding 25 years of service.

Experience You Can Count On

Eldercare Services is nationally recognized for its experienced professionals. Most of our Professional Geriatric Care Managers are certified by the National Academy of Certified Care Managers with the CMC designation. The Executive Director and several of our Care Managers also belong to the National Association of Professional Geriatric Care Managers.

Additionally, we provide Care Management for non-seniors, including advocacy and direct services such as counseling or care giving for adults with developmental disorders, long-standing mental health issues or those younger than 55, whose care is managed by “special needs trusts.”

Professional Geriatric Care Managers adhere to a Code of Ethics and follow Standards of Practice to assure high quality of care is delivered.
