We are currently seeking candidates for the following positions:
Geriatric Care Managers - Walnut Creek
Eldercare Services needs a Professional
Geriatric Care Manager. We are
leaders in the field of geriatric care, offering both Care Management
and non-medical attendant care. We are seeking a health and
human services professional, with a background in social work,
counseling, gerontology, certified care management or home Care
Management.
We are looking for individuals with at least 3-5 years experience in
geriatrics. Experience with family work as well as knowledge
of aging resources and options is a plus.
Opportunity to work one-on-one with our clients and
oversee a client centered care plan that focuses on quality of life. Our
pay and benefits are competitive. Flexible hours. We are a team spirited, caring group and we are looking for
someone with like spirit and commitment to elders. We service Contra Costa, Solano, Alameda, Marin and San Francisco
Counties.
Minimum Requirements:
- Bachelors Degree in a Human Services Field
- Three to Five years Geriatric Care Management Experience
- Familiarity with Computerized Programs
- Team Player and Dedication to Senior Services

Caregivers
- Live-In
Caregiver- 24 Hour Assignments
A person who assists
the client with daily living activities (housekeeping,
cooking) and personal care (dressing, bathing). 24 Live-In
assignments available immediately. Two to 5 days per week.
- Requires:
Compassion, patience, and at least one year experience. Valid
CDL, Reliable Automobile, Current Auto Insurance required. CNA/HHA
Encouraged to apply. Transfer
and Hoyer Lift experience a plus.
- Caregiver-12 Hour Assignments
A person who
assists the client with daily living activities (housekeeping,
cooking) and personal care (dressing, bathing). 12 Hour
Day/Night assignments available immediately. Two to 5 days per week.
- Requires: Compassion, patience, and at least one year experience. Valid
CDL, Reliable Automobile, Current Auto Insurance required.
CNA/HHA encouraged to apply. Transfer
and Hoyer Lift experience a plus.

Personal Assistant-Hourly Assignments

A person who goes with
the client to medical appointments, day programs, outings. May also
help with daily living activities and personal care. Hourly
assignments available immediately.
Flexible schedules available, consistency required.

Auto Insurance. One year previous care giving experience.
C.N.A./H.H.A. encouraged to apply.

Friendly
Visitor-Hourly Assignments
A person who visits the
client and takes him or her on outings. Might read to the client,
play games, watch movies, feed the birds, etc.
Requires:
Compassion, patience, Valid CDL, Reliable Automobile, Current
Auto Insurance. One year previous care giving experience.
Education may be substituted for previous experience.

RN Director of Home Health/Care Management
Eldercare Nursing Services, a new division of Eldercare Services, is seeking a creative, experienced RN to build strong, long-term relationships with clients and professionals. No OASIS - we are licensed for non-Medicare clients. We have manageable paperwork! Client centered nursing and care management. Advocate for clients when they need support; assist physicians with client compliance and allow the older adults to age in place with expert oversight and quality of life. This is at the heart of what we do.
Eldercare Services has been providing expertise in Professional Geriatric Care Management and Caregivers to clients in the Bay Area for the past 18 years. We are a team spirited group and are excited to have the right person who believes in our core values. Join our collective advocacy and bring peace during the last years of our clients lives.
Director will oversee the agency licensing requirements and ensure standards are met at all levels of service.
Position: 30-40 hours per week. On-Call rotation. Pay and benefits are competitive. We believe in flex time - giving your life meaning. We serve most of the East Bay.
Qualifications:
- RN with BA of Higher Degree in Nursing (prefer Geriatrics)
- Three of the past five years in a Health Field Supervisory Role
- Must have a Car and Current Insurance
- Familiarity with Title 22 (non-Medicare)
- The "Right" Spirit
- Experience in Building Teams
