The Care Manager stands by you every step of the way as your advocate, expert and counselwhile at the same time providing supervision, training, continuing education and support to our Caregivers.
Care Management Services
Consultation: Professional guidance to identify issues and explore options, at home or in our offices.
Counseling: Licensed marriage and family therapists and
clinical social workers to help meet emotional needs.
Ongoing Care Management: Overseeing "the big picture" to provide ongoing care coordination, monitoring and advocacy.
In-Home Assessment & Long-Term Planning: Personal visit, written plan and family session to address needed care services, placement options, professional referrals and resources.

Senior Relocation Services : Evaluation and recommendations following personal visit to determine the most suitable placement. Many care facilities extend a 20% discount to our referrals for the first month's rent, representing up to $500 in savings to the client.

a Care Manager?
Caregiving
Hourly Caregivers: Ongoing care to meet client needs (2-hour minimum)
Live-In Caregivers:
12-Hour Shift: Caregivers are expected to have break periods, but are available to respond at all times. Includes up to two hours of care management per month.
24-Hour Shift: Caregivers are expected to have sleep time and break periods, but are available to respond at all times. Includes up to four hours of Care Management per month.
